Social Prescribing for Older Adults - Link Worker
The Town of Bonnyville FCSS is seeking a compassionate and motivated individual to join our team-oriented organization as a Link Worker. Reporting to the Community Services Director or their designate, the Link Worker plays an integral role in bridging the gap between healthcare and social care services.
The Link Worker works directly with healthcare professionals to receive referrals, support clients in accessing non-medical community resources, and help improve overall health, well-being, and quality of life for residents.
Key Responsibilities
- Conduct assessments to identify client needs and develop individualized care and support plans.
- Provide practical and emotional support to clients and their families, ensuring services are tailored to their unique circumstances.
- Collaborate with healthcare professionals, local physicians, community agencies, and organizations to coordinate client care and enhance service delivery.
- Connecting clients with local resources, programs, and supports that promote health and well-being.
- Manage community referrals, ensuring timely follow-up with clients and providing updates and feedback to referral sources.
- Collect, track, and report data to support program evaluation, continuous improvement, and accountability.
- Participate in local community meetings to strengthen partnerships and enhance awareness of available services.
- Engage in provincial communities of practice (e.g., with Healthy Aging Alberta) to share insights and align with best practices.
- Fostering collaboration and consistency in service delivery.
- Stay informed on community resources and emerging opportunities to enhance programs and services for older adults.
Education and Experience
- Post-secondary education in social work, nursing, community development, public health, or a related human services field.
- Experience working with older adults in a health or social services setting is an asset.
- Demonstrated experience in case management, care coordination, or community outreach.
- Familiarity with social prescribing models, community-based health programs, and collaborative inter-professional practices.
Knowledge and Competencies
- Strong understanding of aging, social determinants of health, and community resources available to older adults.
- Excellent assessment, interviewing, and care planning skills.
- Proven ability to build relationships and collaborate effectively with healthcare providers, agencies, and community stakeholders.
- Skilled in data collection, documentation, and reporting for program evaluation and quality improvement.
- Strong communication, problem-solving, and organizational skills, with attention to detail and follow-through.
- Cultural sensitivity and empathy in working with diverse individuals and communities.
- Proficiency with digital tools and databases for record-keeping and reporting.
- Valid driver's license and access to reliable transportation may be required for community outreach.
- The successful candidate will be required to provide a clear Vulnerable Sector Criminal Record Check prior to employment.
Position Available: November 1, 2025
Hours: 0.8FTE and be willing to work shifts that are flexible to meet client needs.
Term: 1 year contract with possibility of renewal.
Starting Wage: $25.00 per hour.
